How to File a Claim   

(FOR DAMAGE OR LOSS TO HOUSEHOLD GOODS AND PERSONAL EFFECTS IN TRANSIT OR STORAGE)

  • The intention of this form is to assist you in submitting a claim should your consignment sustain any loss or damage during the course of transit or storage.

  • Please read this form carefully and ensure that all instructions are followed.

  • Adherence to the instructions will result in your claim being processed more expediently.

  • Any loss/damage must report within 30 days upon delivery of your goods.

    Please contact the Santa Fe station that enrols you in the Group Policy to file for your claim.

  • Substantiation of Claim   

  • The responsibility rests with you as the claimant to fully substantiate your claim.

  • Failure to give notice to the Santa Fe station that enrols you in the Group Policy within 30 days after delivery of the shipment will void coverage.

  • It is mandatory for all supporting documents (see below) to be provided to Santa Fe station that enrols you in the Group Policy within 60 days from the first notice.

    It is essential that full documentation is provided. Failure to submit the following may cause unnecessary delays.

    • Complete the Claim form;
      Download a copy of Claim form.
      (Please submit within 30 days upon delivery of your goods.)

    • Written professional estimates of Repair;
      You must retain all the damaged items until your claim has been settled. Failure to do will result in your claim being prejudiced.

    • Written professional estimates of replacement for missing items or items. Items beyond repair should verified in writing by a professional source;

    • Photographs of all damaged items.

    WARNING: PLEASE NOTE THAT IF YOUR CLAIM IS IN ANY RESPECT FRAUDULENT ALL BENEFITS WILL BE FORFEITED.
